Admissions Criteria and the Referral Process to the Wells House


1. Client’s primary diagnosis must be alcoholism or drug addiction. Co-morbid mental health disorders do not necessarily disqualify an individual as long as the primary diagnosis is chemical dependency (as defined in DSM-IV) and they meet the eligibilty requirements for Wells House modality (as defined in ASAM-PPC-2R Dimension III.1). Clients must be capable of self-administration of medication without supervision. The Wells House is facility for adult men (18 years of age or older).

2. Client has successfully completed an inpatient treatment program (or its equivalent) and have documented proof of chemical abstinence for a minimum of 14 days. A bed-to-bed transfer is required. Although the Wells House has transportation available, we prefer the referring agency bring the client directly to our facility. Arrangements must be coordinated in advance.

3. Client must be referred by the facility or therapist responsible for their care while still in treatment. Once initial contact is made, instructions will be explained on how a potential client can meet the eligibility requirements and a time will be agreed upon for an interview - either in-person or by telephone.

3. Client must have the ability to understand the philosophy of the Wells House concerning recovery from addiction.

4. Client must be willing to abide by the house rules and policies.

5. Client must have a documentation of a health examination and TB test prior to admission and be able to set up and receive needed and/or adequate medical care in the surrounding area. Case management assistance is available from staff, where warranted. Client must have a minimum of 3 days medication with accompanying written 30-day prescriptions to allow adequate lead time to insure continuity of care when first arriving.

6. Client must be physically and mentally able to participate in the program components. The Wells House program is designed for clients to work in the community during the day and to be involved in recovery-related activities at night and on weekends and holidays. Clients must be able to seek and find employment. Case management is provided to help our clients find a job or to seek adult education or vocational training if needed. If a client has a pre-existing disability, alternate volunteer service may be required.

7. Client is able to express, to the satisfaction of the clinical staff during the prescreening, an honest appraisal of their addiction as well as a willingness to make needed lifestyle changes and a willingness to participate in the structured components of the house. 8. Clients currently receiving pharmacotherapy (methadone or buprenorphine) are eligible as long at they meet our other admissions criteria. In these circumstances advance planning must be coordinated well in advance to their admission.

9. Clients with a history of violence may be excluded if deemed inappropriate for our level of care.
