Mindy can see patients in both Hagerstown and Frederick! Mindy is a clinical social worker with many years’ experience working with clients who struggle with mental health challenges such as anxiety disorders, depression, bipolar disorder, co-occurring substance use, trauma, suicidality, self-harm, grief, and difficulties related to major life changes. She uses a variety of methods to help people create change in their lives, such as Cognitive Behavioral Therapy, Short Term Solution Focused Therapy, Trauma-Informed therapies, and Psychodynamic therapy. Mindy works with adults, adolescents, and children. In 2001, she graduated from Shepherd University with her bachelor’s degree in social work, and in 2006, she obtained her master’s degree in social work from the University of Maryland at Baltimore. She has worked for Wells House as a therapist and Clinical Director since 2022.

Kathleen Gladmon, LMSW
Kate provides counseling services to adults experiencing a range of challenges, including anxiety, depression, grief, addiction, and concerns related to self-esteem. Kate earned her Master’s Degree in Social Work in 2021 and has been employed at Wells House since 2025. Prior to joining Wells House, she gained valuable experience working in hospice care and skilled nursing facilities.

Libby Palumbo, LCSW-C
Libby is a clinical social worker with extensive experience in working to support individuals living with substance use disorders, co-occurring mental health conditions, and trauma. Her passion is to help clients break cycles of self-destruction, develop emotional regulation skills, and rebuild a meaningful life in alignment with their values. Libby integrates evidence-based practices with intuitive and somatic techniques to support healing on every level: mental, emotional, physical, and spiritual. Her approach is trauma-informed, nonjudgmental, and rooted in a deep respect for each person’s unique path to recovery and wellbeing.

Joe Pignone
Joe serves as the Mental Health Coordinator at Wells House Counseling and Wellness, where he plays a central role in supporting both patients and clinical staff. In this position, Joe manages multiple aspects of patient care coordination, including completing referrals for incoming Outpatient Mental Health Clinic (OMHC) patients, scheduling and rescheduling appointments, conducting reminder calls, and collecting essential patient data. In addition to his coordination duties, Joe oversees the weekly schedule for the program’s Psychiatric Nurse Practitioner (NP), ensuring seamless collaboration for patients who require medication management. He also facilitates medication refills throughout the week, helping maintain continuity of care for individuals receiving psychiatric treatment.

Annie is a licensed professional counselor with experience working with diverse population of individuals with varied needs, including those on the Autism spectrum; members of the LGBTQ community; individuals with personality disorders, children and adolescents requiring inpatient care, and individuals with substance use disorders, history of incarceration and homelessness.

Jennifer’s path to becoming a therapist wasn’t linear—and she believes that’s part of what makes her relatable and effective. She first explored psychology while earning her bachelor’s degree from the University of Virginia, then spent 17 years in the corporate world before returning to her true calling during the pandemic. She earned her master’s in social work from the University of Maryland and became an LMSW in 2025.

Denise earned her Master of Social Work degree from Salisbury University and obtained her LMSW in 2026. She believes that healing happens when individuals feel heard, respected, and empowered to create meaningful change in their lives. Her approach is compassionate, collaborative, and person-centered, meeting each client where they are in their journey.
